APPOINTMENTS


Patients are seen by appointment only. New patients will be asked to complete several forms and to sign appropriate authorizations. As a returning patient you may be asked to update forms. Go to our patient portal to download and complete our forms.

When you arrive for your appointment please advise our reception staff of any changes to your address, phone number, insurance coverage or other billing information. If you have had any x-rays and diagnostic studies (MRIs, CTs or EMGs) taken prior to your appointment, please make sure that you bring them with you or have them sent prior to your appointment at our office.

CANCELING APPOINTMENTS


We would appreciate at least 24 hours notice when changing or canceling your appointment. A charge will be added to your account for failed appointments.
