Orthopedic Associates accepts cash, check, VISA, MasterCard, Discover Card, American Express and ATM. Our Business Office is happy to discuss co-payments, deductibles, financial arrangements and any difficulties concerning your bill.



Private Insurance: If you have private insurance, please bring your insurance card and address where claims should be submitted. Please be prepared to pay your co-pay at the time of your visit. You are responsible for your deductible and co-insurance and any non-covered services and supplies.

Authorizations and Referrals: If your insurance requires a referral and/or authorization, please make sure we have the information before you come for your appointment or we may have to reschedule. Please discuss any difficulties concerning authorizations with our reception staff.

Medicare: Our physicians accept Medicare assignment. We will bill your Medicare and supplemental insurance if applicable. Your deductible, co-insurance and payment for non-covered services are your responsibility.

Workers' Compensation: If we are billing worker's compensation for you we will need your compensation information. This includes the date of injury, the name, address and telephone number of both your employer and compensation carrier.

Private Pay/Uninsured: A $400 deposit/payment is required at the time of your initial visit. Payment in full is expected at each future visit. Please talk to our Business Office about discounts and other payment arrangements.

Completing Insurance forms: We will be happy to complete insurance forms. Insurance plans vary greatly and you should understand that you are responsible for any amount not covered by your insurance plan.
